The following are descriptions / instructions of the NON-PS2 fields (Intake Tab)

Field: Application Date
Instructions: Enter the date the client requested benefits. If you are modifying existing information about a case, this field will display the date the application was taken. This entry appears on the printed application form, but does not change any PS2 fields.

Field: Interview Date
Instructions: Enter the date the client’s interview was conducted.

Field: Interview By
Instructions: Enter the name of the worker who performed the initial interview with the client. This is not necessarily the ongoing worker.

Field: Interpreter Needed
Instructions: Check this box if the client does not speak English and needs an interpreter. Mark this box even if the client provided their own interpreter so the next worker will know that the client does not speak English very well.

Field: Initial Questions Sub-Tab: Anyone lived in any other in the last 12 Months?
Instructions: Check “Yes” or “No” to indicate whether anyone in the household has lived in any other states in the last 12 months.

Field: Initial Questions Sub Tab: If yes, what states?
Instructions: Indicate in which state(s) other than Oklahoma any household member has lived in the last 12 months.

Field: Initial Questions Sub Tab: Anyone receive benefits in other states?
Instructions: Check “Yes” or “No” to indicate whether anyone in the household received benefits in another state in the last 12 months.

Field: Initial Questions Sub Tab: If yes, who received benefits in another state?
Instructions: Enter the names of anyone who received benefits in another state in the last 12 months.

Field: Initial Questions Sub Tab: What states?
Instructions: Enter the name of the state(s) in which the client received benefits within the last 12 months. The state of former residence must be contacted to determine if benefits are still being received in that state to avoid overlapping of benefits.

Field: Initial Questions Sub Tab: What type of benefit?
Instructions: Check the box representing the type(s) of benefits the client last received. This helps you track what the client may already be getting.

Field: Initial Questions Sub Tab: Identity checked
Instructions: Mark the correct choice when the identity of the applicant has been verified. If the client is already known to the worker, no verification needs to be provided.

Verification of identity is required for all programs and is never postponed. For Medicaid recipients the Identity Verification Status field must also be updated.

Field: Initial Questions Sub Tab: Date of last benefit
Instructions: Enter the date of the last benefits the client received.

Field: Initial Questions Sub Tab: Still receiving benefits?
Instructions: Mark this box if the client reports they are currently receiving any benefits either from OKDHS or any other state or Tribe.

Field: Initial Questions Sub Tab: Plan to stay in OK?
Instructions: Mark here if the client is either a resident of Oklahoma or plans to become a resident. Only residents of the state of Oklahoma are eligible for benefits through the OKDHS. Visitors who do not plan to remain are not eligible.

Field: Initial Questions Sub Tab: Is anyone in your household a fleeing felon?
Instructions: Enter a check if the answer is yes.
See SNAP: Disqualified household members 340:50-5-10.1.
This affects the ability of the fleeing felon to receive food benefits. You may not report a fleeing felon to the police, the information provided is confidential; however you are encouraged to contact OIG regarding a fleeing felon.

Field: Household Members Sub-Tab: Still living in home?
Instructions: Indicate by choosing “Yes” or “No” whether the person in question is still living in the home.

Field: Household Members Sub-Tab: Ever received Tribal TANF?
Instructions: Indicate by choosing “Yes” or “No” whether the person in question has ever received Tribal TANF. Concurrent receipt of Tribal TANF and DHS TANF is not allowed.

Field: Earned Income Sub Tab: Current employer
Instructions: This field shows all current employers for this person. A current employer listed on the Income Tab with a date in the begin date and a “blank” employment end date. If no employer is on FACS for this person, the field will not display.

Field: Earned Income Sub Tab: Is this person still employed here?
Instructions: This is one of three questions/statements that must be answered to address this person’s employment status. This question must be answered yes if the client is employed. If the current employer is not shown in the Current Employer field then you will need to add them. After adding the new employer, hitting the next button at the bottom of the screen will add them to the list. You can then answer this question yes.

Field: Earned Income Sub Tab: When did their employment end?
Instructions: Enter the last date of employment if the client is no longer at this employer. This will also populate the Employer End Date on the Earned Income Tab. Use best available information to determine the end date of the employer.

Field: Earned Income Sub Tab: Employer’s name
Instructions: Enter the name of the client’s current employer if not already displayed in the Current Employer drop down list. This will also cause the new employer to show the Earned Income Tab.

Field: Earned Income Sub Tab: This person is self employed
Instructions: Choose “Yes” or “No” to indicate if the person in question is self-employed. This is one of three questions/statements that must be answered to address this person’s employment status. At certification, one of the three employment status fields must be a yes for every food stamp household member whose income will be considered in this case or FACS will issue an internal validity check.

Field: Earned Income Sub Tab: This person is unemployed
Instructions: Choose “Yes” or “No” to indicate if the person in question is unemployed. This is one of three questions/statements that must be answered to address this person’s employment status. At certification, one of the three employment status fields must be a yes for every food stamp household member whose income will be considered in this case or FACS will issue an internal validity check.
