Support Services: Maintenance Requests

What type of requests should be submitted?

Facilities Management Unit (FMU) supports DHS owned building and workstations. If request is janitorial or grounds related contact the local contract monitor. Submit FMU maintenance requests for HVAC, plumbing, electrical, workstations/cubes, roof, window/blinds, creatures/other. For door code lock changes please give 24 hour notice if applicable: i.e: employee resignation, termination or security protocol breach.

Who can make a request?

All requests for assistance should go through the Field Manager and the Support Services Coordinator (SSC) of your area. To know who is assigned to your area, please see  SSC map. The SSC will then submit a request to FMU inbox.

Request Email Details and Example:

To: FMUBldgMaintenance@okdhs.org

Subject: “County Name”/”The Issue”

Email Body: Details of the issue & where it occurring. Backup contact person if necessary.

What happens after a request is made?

After your request is submitted, you should hear back from FMU within four hours. If you do not hear back within this time frame, follow up to FMUBldgMaintenance@okdhs.org by forwarding your initial request email.

Request completion

If you receive an email notifying you that your request is closed and your request is not complete, “reply all” with the outstanding issues.
