An individual must provide verification of identity when requesting an EBT card in person at a local OKDHS office.
The best form of identification is a photo ID such as a driver’s license or state issued ID.
If the client does not have a photo ID with them, the AFS employee can check in the OnBase imaging system to determine if a clear and valid photo ID exists. The image must be detailed enough to recognize the picture in the photo ID as the same person that is in the office.
If a photo ID is not available, an alternate form of identification may be requested from the client, such as a work or school ID, health or social service program ID, paystub, birth certificate, or voter registration card.
If a photo ID or alternate form of identification cannot be provided by the client, staff will not request an EBT card and should refer the person to call Conduent for the card to be mailed.
Collateral contacts are an acceptable method to verify identification; however, this should be the method of last resort. If a collateral contact is the only means of verifying identification, the person providing the collateral contact must provide photo ID and the client must verify verbal identifying information on the case.
Use of verbal identifying information, such as asking for the date of birth or the last 4 digits of the social security number, are not an alternate form of identification for an over-the-counter EBT card. This information should be used in conjunction with the alternate form of identification to complete the verification of identity but is not sufficient with only the verbal information.
