TANF: ELIGIBILITY – What Forms Are Needed for a Review?

What forms are required to complete a TANF Review (child only)?

All forms must be completed and imaged into Onbase

If using paper application forms:

  • Completed Request for Benefits (PS-1)
  • Completed Eligibility Information for Benefits (FSS-1) from FACS
  • Signed Responsibilities and Signature for Benefits (FSS-1B)
  • Lump Sum Payments (FSS-C-4)
  • Absent Parent Information Sheets (FSS-AP-1-A) if any new information is available of the parents of the child
  • Cooperation Agreement and Request for Good Cause, 08TA012E (C-9)

What forms are required to complete a TANF Review?

All forms must be completed and imaged into Onbase

If using paper application forms:

  • Completed Request for Benefits (PS-1)
  • Completed Eligibility Information for Benefits (FSS-1) from FACS
  • Signed Responsibilities and Signature for Benefits (FSS-1B)

If using online proxy, the OKDHSLIVE application will be in Onbase already. The above forms are not necessary.

  • Updated TANF Work/Personal Responsibility Agreement (TW-2)
  • Lump Sum Payments (FSS-C-4)
  • Absent Parent Information Sheets (FSS-AP-1-A) if any new information is available of the non-custodial parent of the child
  • Cooperation Agreement and Request for Good Cause, 08TA012E (C-9)
