ELIGIBILITY – What Forms Are Needed for a Review?

All forms must be completed and imaged into Onbase

If using paper application forms:

Completed Request for Benefits (PS-1)
Completed Eligibility Information for Benefits (FSS-1) from FACS
Signed Responsibilities and Signature for Benefits (FSS-1B)
Lump Sum Payments (FSS-C-4)
Absent Parent Information Sheets (FSS-AP-1-A) if any new information is available of the parents of the child
Cooperation Agreement and Request for Good Cause, 08TA012E (C-9)

All forms must be completed and imaged into Onbase

If using paper application forms:

If using online proxy, the OKDHSLIVE application will be in Onbase already. The above forms are not necessary.

Updated TANF Work/Personal Responsibility Agreement (TW-2)
Lump Sum Payments (FSS-C-4)
Absent Parent Information Sheets (FSS-AP-1-A) if any new information is available of the non-custodial parent of the child
Cooperation Agreement and Request for Good Cause, 08TA012E (C-9)
