The following guidelines apply to any accident that happens on DHS property, whether it be a client or worker.
- Field Manager or Supervisor ensures prompt medical treatment
- Employee reports it immediately and completes accident report form
- Investigate and gather witness statements
- Submit forms to Risk Management HRMD.RISKMGMT@okdhs.org
Completed forms are scanned to STO.HRMD.RISMGMT@okdhs.org and then mailed to the Human Resources Management Division.
