The following guidelines apply to any accident that happens on or off DHS property while an employee is on duty.
- FM or Supervisor ensures prompt medical treatment using initial treatment form and Workers Comp in-network providers
- Employee reports incident/accident immediately and completes DHS Workers Comp App along with supervisor
- Employee will complete SLIR
- Submit any additional information (provider notes/discharge/release paperwork, LOE, questions, etc.) to Workers Compensation & Drug Testing hrm.workerscompensation@okdhs.org
