If someone had forgotten their OKDHSLive password, from the login page select “Forgot your password?”.
Enter the email address/User ID, last name and date of birth associated with the OKDHSLive account, and click continue.
The next page will display two of the security questions that were answered when the account was created. Answer these questions to reset the password and continue onto the site. Be sure to answer the questions just as they were answered when the account was created. These questions are case sensitive so if there is capitalization or punctuation it must be included when attempting to reset the password.
If these questions cannot be answered correctly, the user will need to call (405) 522-5050 and select the option for password reset so an agent can assist in resetting the password. If the user is unable to answer the questions with the OKDHS agent, a new account will need to be created.
