Interviews are conducted for the purpose of assessing the needs of the individual making application to determine if additional services are needed and to obtain or request the necessary income and resource documentation to determine financial eligibility for the services being requested.
The interview for Long-Term Care services can be conducted in the county office, on the telephone, in a nursing facility or in client’s home. The interview can be conducted with the client or client’s representative, which can be an adult family member, a friend, a legal guardian or the nursing facility administrator.
If the interview is being conducted in the county office, the Rights and Responsibilities video must be shown and the FSS-1-B form must be signed. If the interview is conducted on the telephone or outside the county office, the Rights and Responsibilities must be gone over verbally and the FSS-1-B form must be mailed for signature. Voter registration must always be offered.
The interview should be conducted using the FACS Interview Notebook moving from tab to tab to gather all the information needed to make an eligibility decision. If additional documentation is needed upon completion of the interview, an ADM-92 is given to or mailed to client or their representative.
