Child Care Subsidy: How to Add Child to Child Care

The following instructions outline the process for adding child to an open child care case:

On the Intake tab in Interview Notebook:

  1. Click on the “HouseHold Members” button
  2. Click on the “Add New Household Member” button
  3. Enter Last Name
  4. Enter First Name
  5. Enter M.I.
  6. Enter Date of Birth:
  7. Click OK

On the Household Tab, Interview Notebook

  1. enter the following for added household member:
    • SSN
    • Verified SSN
    • Sex
    • Marital Status
    • Relation to Payee / Spouse
    • Ethnicity & Race Data
    • Citizen/Alien Data
    • Living Arrangement
    • Education Data
  4. Reason= Leave Blank
  5. Effective Date = Enter date in which child care is to begin.

On the Auth. Daycare Tab, Eligibility Notebook:

  1. Person Name = Select child to be added
  2. Enter the following:
    • Contract Number
    • Type of Care
    • Action Taken = OPEN
    • Reason = Enter appropriate reason
    • Child Care Req Date = Enter date child care is to begin
    • Begin/Change Date = Enter date child care is to begin
    • Unit Type 1: (Daycare Plan) = Enter Appropriate Unit Type
    • Number of Units = Enter appropriate number of units
    • Notice Indicator = Application Approval

On the Child Care Tab, Eligibility Notebook:

  1. Action Taken = Change of Information
  2. Effective Date = Enter appropriate Effective Date
  3. SAVE the Case and PUSH Case completely through
