Long Term Care (LTC): Process for Closing Cases (Death)

The following outlines the procedures for closing Long Term Care cases in the event that the client has died.

ADvantage or SPPC:

The worker emails the nurse to advise the client has died and provides the date of death. The nurse will close the authorization through ELDERS. The worker waits for the nurse to close the authorization.

Once that has happened, the worker closes the case (B or D section – whichever is appropriate for reason code “Death-No Notice Generated [01]”). The worker should not close the case until after the RN has closed the authorization.

Nursing Facility:

The worker closes the authorization for reason code (1) Death of the Client. The end date will be the date of death. After the authorization is closed, the worker closes the case (B or D section – whichever is appropriate for reason code “Death – No Notice Generated [01]”).

Note: As the reason code indicates, the client’s family does not receive a computer generated notice when the B or D section is closed on death. However, the provider will receive a closure notice from the authorization section.
