Overview

The Medically Fragile program is a home and community-based alternative to placement in a hospital and/or skilled nursing unit of a nursing facility to receive Medicaid-funded assistance for care. Medically Fragile Waiver provides in-home services to eligible adults aged 19 years and older who have a life-threatening condition that requires frequent medical supervision, frequent administration of specialized treatment, or who are dependent on medical technology. Eligibility Requirements:

  • Be at least 19 years of age or older;
  • Meet hospital and/or skilled nursing care;
  • meet financial eligibility for long-term care (Appendix C-1, Schedules VIII.B.1 and D); Also refer to article LTC: Look-Back Period for All Assets for LTC
  • have UCAT scores that indicate skilled level of care (SNF) or hospital level of care but the individual prefers to remain at home.

New Applicants

If someone contacts the agency and wants to apply for Medically Fragile, the in-home assistance online application can be used with Medically Fragile selected.
