Closure of SPPC or ADvantage Waiver services between the 1st deadline & end of month
The Harmony system will automatically close for the appropriate negative action date regardless of the time of the month in which the closure action is made.
Client moves from one nursing facility to another after medical determination
Once medical eligibility has been determined for NF level of care; eligibility is valid regardless of which nursing facility the client chooses to reside. No new application/medical decision is needed if a medical decision was made for the 1st admission and period of absence from the NF was less than 90 days.
- If absence from the facility was greater than 90 days and the client was not receiving skilled or hospital care during the absence from the facility, a new NF application should be submitted.
- If client was discharged from NF to home on ADvantage or SPPC then returned to the same or another NF, regardless of timeframe, a new NF application is required so that the services may be closed appropriately in Harmony.
Since Harmony does not manage the NF authorizations; worker will close and certify appropriate authorizations when client moves to a different facility.
