There are three forms required for all paper applications:

1. 08MP001E PS-1   Request for Benefits
   This is the application that the client completes and signs to request benefits.
2. 08MP003E FSS-1-B  Responsibilities and Signature for Benefits
   This form advises clients of their rights and responsibilities for all programs, and it requires a client signature.
3. O8MP002E FSS-1  Eligibility Information For Benefits
   This form is generated in FACS after the interview is completed, using information that auto-populates from the blocks in FACS. Workers can image this form directly into their packets from FACS.

There are additional forms that may be required when completing applications:

• 08AD091E Interview Notice
  If an interview needs to be scheduled with a client, workers can mail this form, allowing time for mail, to schedule the interview via phone or in office.
• 08AD092E ADM-92 Client Contact and Information Request
  If a worker needs to request verification to complete an application, that worker must complete an ADM-92, allowing for at least 10 days for the client to provide verification. Workers must also send this form if they use the SEO process to waive verification.

For Benefit Reviews (MCRs), the following form is required:

• 08MP004E FSS-BR-1 Renew My Benefits
  The PS-1 and FSS-1-B can be submitted in place of the FSS-BR-1, but the FSS-BR-1 cannot be used for anything other than reviews. (It can NOT take the place of an application)
