Aged, Blind, Disabled (ABD): Interview Process

Interviews are conducted for the purpose of assessing the needs of the individual making application to determine if additional services are needed and to obtain or request the necessary income and resource documentation to determine financial eligibility for the services being requested. The interview for ABD services can be conducted in the county office, on the telephone or in the client’s home. The interview can be conducted with the client or client’s representative, which can be an adult family member, a friend or a legal guardian.

The worker, whether in county office or by phone, should attempt to complete a proxy through OKDHSlive.org. During this process, the worker must read the Rights and Responsibilities (R&R) to the client and get acknowledgement of agreement to this prior to submitting the application. It is also necessary to offer voter registration.

If for some reason, OKDHSLIVE is not available, it is required to have the FSS-1-B (Rights and Responsibilities) to be signed which will act as the application.

The interview should be conducted using the FACS Interview Notebook, using  Case note generator and inserting the template into the case information tab of FACS. If additional documentation is needed upon completion of the interview, an ADM-92 is given to or mailed to client or their representative.
