Please send the following letter to oil/gas companies via email to request information regarding mineral interests. (Please be sure your email includes a signature block that includes your title, organization information, and contact information.)

Owner Relations,

John Doe, owner number ########, has given permission to the Oklahoma Department of Human Services (OKDHS) to obtain information regarding any and all mineral interests in which he/she has an interest in accordance with the attached release authorization. OKDHS is aware that in the past John Doe has received royalties from and/or leased a mineral interest to your company.

Please provide the following information in regards to the above-referenced owner:

For royalty-producing mineral interests:

1. legal description of the property in which the mineral interest is located;
2. total gross royalties produced over the past 12 months;
3. 1099 for the most recent year of production; AND
4. amount of funds held in suspense.

For mineral interests leased, but not producing:

1. 1099 issued by your company in the year the bonus was paid or the gross amount of the lease bonus paid;
2. legal description of the property in which the mineral interest is located;
3. anticipated end date of lease; AND
4. total number of net mineral acres owned by the referenced owner.
