How to Access Harmony from the InfoNet

Use the following steps to access Harmony from the InfoNet.

Log into SharePoint and access the main page of the InfoNet.

Hover over the Service Divisions Button and scroll down to the Adult and Family Services button and click to access the page.

Hover over the Online Services button to access the drop-down menu.

Scroll down to the Harmony button to access the Harmony options drop-down menu.

Note

For Nursing Facility applications and reopens of ADvantage and SPPC (closure of services was less than 90 days), use the Harmony Referral Form
To submit a new application for ADvantage/SPPC for a new applicant or for an individual requesting to transition from SPPC to ADvantage or ADvantage to SPPC; use the Harmony Application for In-Home Assistance.
